This Second Edition of Version 5 of the Allen Cognitive Level Screen is available in a 117-page, spiral-bound version. The ACL Screen is a theory-, performance- and evidence-based assessment associated with the Cognitive Disabilities Model (CDM). It is for use with persons with known or suspected cognitive impairments that may impact safe and successful performance of everyday activities.
The Screen provides a quick estimate of global functional cognitive capacity, including learning potential, and functional performance abilities that require that capacity. This estimate is inferred from observation of a person using the administrator’s verbal and demonstrated directions to problem-solve, learn, and complete three visuospatial-motor stitching tasks of increasing cognitive complexity. Screen scores are criterion-referenced to the Allen Cognitive Scale of Levels and Modes of Performance and should be verified with additional functional cognitive assessments as part of a comprehensive evaluation. The ACL Screen is available in three forms:
Standard Allen Cognitive Level Screen (ACLS-5) for general use
Large Allen Cognitive Level Screen (LACLS-5) for persons with vision or hand function problems
Disposable LACLS (LACLS[D]-5) for persons with whom infection control is required.
Administration Time: The Screen is untimed and takes approximately 15-30 minutes to administer.
Who May Administer, Score and Interpret Scores:
Occupational therapy practitioners and other healthcare professionals within the scope of their practice acts and in compliance with role delineations defined by their professional associations and applicable licensing agencies, and who are:
Familiar with the rationale for and use of the three forms of Version 5
Trained/mentored in use of the CDM and in the administration/scoring/verification/interpretation/reporting of the ACL Screen, Version 5
Experienced in working with persons with temporary or permanent cognitive disabilities or impairments.

EVA40402 Large Allen Cognitive Level Screen (LACLS): This is a large form of the ACLS for persons with impaired vision or hand function who have difficulty using the standard ACLS leather piece. Research studies have demonstrated correspondence with the ACLS. Includes: 1 pre-punched 6x7 inch rounded, tan leather rectangle, 1 shoelace with plastic tips on each end, 2 large brass threaded locking needles, 1 hank of wide leather lace with 2 visibly distinct sides. EVA40406 Disposable LACLS-5(D) 4 Pack: Developed for use where strict infection control procedures must be observed. This fully disposable tool is designed for a single use or serial use with one individual, after which it should be discarded. Materials to set up four screening tools are provided. Each pack includes: 4 disposable chipboard forms, pre-cut shoelace lengths, 4 pieces of 60” lacing to be cut in half for use with each tool, set up instructions unique to the tool and contact information to participate in a feedback survey. Needles are not used with the (LACLS[D]). The “front” of the tool is indicated by the ACLS & LACLS Committee Approved stamp. Follow administration protocol in The Manual for the Allen Cognitive Level Screen-5 (ACLS-5) and Large Allen Cognitive Screen-5 (LACLS-5).
