The Allen Cognitive Level Screen Version 5 (ACL-5) is a standardized assessment rooted in theory, performance, and evidence-based methodologies that provides a quick estimate of global functional cognitive capacity, including learning potential and functional performance abilities. This estimate is inferred from observation of a person using the administrator’s verbal and demonstrated directions to problem-solve, learn, and complete three visuospatial-motor stitching tasks of increasing cognitive complexity. Developed within the framework of the Cognitive Disabilities Model (CDM), this assessment is designed to evaluate functional cognition among individuals with known or suspected cognitive impairments. Screen scores are criterion-referenced to the Allen Cognitive Scale of Levels and Modes of Performance and should be verified with additional functional cognitive assessments as part of a comprehensive evaluation. It is particularly invaluable for healthcare practitioners, especially occupational therapists, in gauging cognitive abilities crucial for safe and successful performance of daily activities.
The ACL Screen is available in three forms:
Standard Allen Cognitive Level Screen (ACLS-5) for general use
Large Allen Cognitive Level Screen (LACLS-5) for persons with vision or hand function problems
Disposable LACLS (LACLS[D]-5) for persons with whom infection control is required.
Age: The ACL Screen Version 5 is suitable for individuals across various age groups who present with cognitive impairments impacting their functional abilities. It is suitable for ages adolescence+.
Administration Time: The Screen is untimed and takes approximately 15-30 minutes to administer.
How To Use It:
Administering the ACL Screen Version 5 involves observing individuals as they tackle three visuospatial-motor stitching tasks of increasing cognitive complexity. These tasks include the Running Stitch, Whipstitch, and Single Cordovan Stitch, performed on either a leather or chipboard rectangle.
Through verbal and demonstrated directions, the administrator evaluates problem-solving skills, learning potential, and overall functional cognitive capacity.
Who May Administer, Score and Interpret Scores:
Occupational therapy practitioners and other healthcare professionals within the scope of their practice acts and in compliance with role delineations defined by their professional associations and applicable licensing agencies, and who are:
Familiar with the rationale for and use of the three forms of Version 5
Trained/mentored in use of the CDM and in the administration/scoring/verification/interpretation/reporting of the ACL Screen, Version 5
Experienced in working with persons with temporary or permanent cognitive disabilities or impairments.

This is a large form of the ACLS for persons with impaired vision or hand function who have difficulty using the standard ACLS leather piece. Research studies have demonstrated correspondence with the ACLS. Use with new Manual for ACLS-5 and LACLS-5 (sold separately). Needles are not used with the (LACLS[D]). The “front” of the tool is indicated by the ACLS & LACLS Committee Approved stamp.
